Lessons Learned System Design & Facilitation.

Build a lessons-learned system that actually changes how work is done — one where lessons are captured, validated, and applied, rather than stored and ignored.

Knowledge Management Services

The Situation — When Clients Come to Us

Organizations come to us for lessons learned work when:

  • A lessons-learned database exists but is unused — either nothing is being captured, the database is full of low-quality entries, or good lessons are being collected but never applied to future work — and the same mistakes are being made on repeat
  • Project-intensive organizations in engineering, construction, defense, oil and gas, pharmaceuticals, professional services, and government programs where learning from experience is a safety, compliance, or performance requirement — but current processes are not delivering real behavioral change
  • Organizations implementing ISO 30401 where a formal, auditable lesson-learning process is a required component of the KM management system
  • Organizations that have tried to implement lessons-learned programs before and found them unsustainable — because the process was too burdensome, the lessons were too generic, or the governance that would apply lessons to future work was never designed

What We Do — Our Approach

  1. // Phase 1

    Lessons Learned Diagnosis

We diagnose the specific failure in the current lesson-learning cycle — identifying whether the problem is in lesson capture (sessions not held), lesson quality (poor entries), lesson storage (inaccessible or unsearchable), or lesson application (captured but never used) — and design the intervention specific to the root cause.

  2. // Phase 2

    Lesson-Learning Process Design

We design the integrated lesson-learning process — the full cycle from lesson identification through capture, validation, storage, communication, and application. We design the Before, During, and After (BDA) review protocols appropriate to the organization's project and operational cycles.

  3. // Phase 3

    Lesson Quality Framework

We design the lesson quality framework — defining what makes a good lesson, the validation and editorial review process that filters out poor-quality entries, and the standards that ensure lessons are actionable rather than anecdotal.

  4. // Phase 4

    Repository Design & Implementation

We design and implement the lesson repository — information architecture, metadata schema, search configuration, and the platform integration that makes lessons findable at the point of need in the workflow where they will be applied.

  5. // Phase 5

    Facilitation & Governance

We facilitate the organization's first lesson-learning sessions — After Action Reviews, retrospectives, project close-out reviews — providing the facilitation and the practical experience transfer that enables internal facilitators to run the process independently. We design the governance model: lesson ownership, review cadence, application checkpoints, and metrics.

What You Get — Deliverables

  • Lessons-learned process design — full cycle from identification through application, including BDA review protocols
  • Lesson quality framework — standards, validation process, and editorial guidelines
  • Lesson repository design and implementation — information architecture, metadata, and search configuration
  • After Action Review and project retrospective facilitation (direct facilitation by Iknow)
  • Internal facilitator training and facilitation guides
  • Governance model — lesson ownership, review cadence, application checkpoints, and performance metrics
  • ISO 30401 compliance documentation (where applicable)
  • Lesson-learning performance baseline and improvement tracking

A lesson captured and never applied is just a document.

Iknow designs and facilitates lessons-learned systems that actually change how work is done — with a methodology grounded in decades of KM implementation experience across every major sector.
